In this article, we’ll explore upper blepharoplasty, one of the most effective procedures for correcting droopy eyelids and rejuvenating the upper eye area, especially in middle-aged and older patients.
Upper blepharoplasty, also known as double eyelid surgery, is a procedure that removes sagging upper eyelid skin and recontours the surrounding structures to create a more open, youthful appearance. It improves not only the aesthetic but also functional issues, such as visual field obstruction caused by hooded or overhanging eyelid skin.

As we age, the upper eyelid skin becomes loose and starts folding over, creating natural-looking creases—similar to how long pants wrinkle at the ankle when they’re too long.
However, if the levator muscle (which lifts the eyelid) weakens, you may not develop visible folds at all—or if you do, the sagging skin may cover the crease completely.
Upper blepharoplasty can be thought of as a middle-aged version of double eyelid surgery, performed with the goal of removing excess skin while also creating or restoring a crease. Compared to younger patients, the amount of skin resected is typically much greater.

In middle age, the levator muscle often weakens with age. To achieve more natural and longer-lasting results, we may combine upper blepharoplasty with ptosis repair.

Trying to fix all sagging in one procedure is not ideal. Over-removal of eyelid skin can result in an unnatural crease or “sausage fold” appearance that is difficult to correct later.
In such cases, combining upper blepharoplasty with brow lift or sub-brow lift can deliver more balanced, aesthetic results.

Most patients can return to normal activities within 7–10 days after surgery. Recovery time varies depending on age, skin elasticity, and health conditions like diabetes or hypertension.
Even after returning to work, mild bruising or swelling may persist for a few more days, so we recommend planning important meetings or events accordingly.
A successful result requires a customized surgical plan based on the patient’s anatomy, skin type, and level of eyelid drooping. Aggressive skin removal may not deliver natural results.
